When you enroll your child in Hazel’s telehealth programs, we will ask for their health insurance information. If your child has insurance, we can help verify coverage for Hazel services. If your child does not have insurance, we can help! Your family might qualify for Medicaid/CHIP.

What is Medicaid? 

Medicaid is a government health cover for eligible people, including many children and families. Eligibility rules vary by state.

What is CHIP?

CHIP stands for the Children's Health Insurance Program, which covers children under 19 in families who earn too much to qualify for Medicaid but still need affordable coverage. 

If you do not qualify for Medicaid or CHIP, you may still have options.

  • Visit healthcare.gov to shop for private marketplace plans. Depending on your income and household size, you may qualify for financial assistance that lowers monthly premiums and out-of-pocket costs.
  • Community Health Centers — Federally Qualified Health Centers (FQHCs) offer care on a sliding fee scale based on your income. Find one near you at findahealthcenter.hrsa.gov.

To help you get started you'll find insurance options below organized by states in which we currently partner with districts. Every state has their own unique Medicaid / CHIP program, so the names may look different depending on where you live.

Arizona

  • Medi-Cal (Medicaid/CHIP) — Free or low-cost coverage for eligible children, regardless of immigration status.
  • Covered California (State Exchange) — Shop for private health plans with potential premium subsidies.
Colorado
  • Health First Colorado (Medicaid) and Child Health Plan Plus (CHP+)
  • Connect for Health Colorado (State Exchange) — Shop for private plans with potential financial assistance.
Florida
  • Florida KidCare (Medicaid/CHIP) — Affordable coverage for children including the following plans: Medicaid, MediKids, and Florida Healthy Kids.
Georgia
  • Georgia Medicaid / PeachCare for Kids (CHIP)

  • All Kids (Medicaid/CHIP)
  • Get Covered Illinois (Marketplace) — Shop for private plans with potential premium subsidies.

  • Maryland Health Connection (State Exchange) — Compare and enroll in private health plans with potential financial assistance.

  •  MassHealth (Medicaid/CHIP) 
  • Massachusetts Health Connector (State Exchange) — Shop for private plans with potential subsidies or apply for ConnectorCare — low-cost plans with reduced premiums and copays for families who don't qualify for MassHealth but still need affordable coverage.

  • Get Covered New Jersey (State Exchange) — Shop for private health plans with potential financial assistance.
