When you enroll your child in Hazel’s telehealth programs, we will ask for their health insurance information. If your child has insurance, we can help verify coverage for Hazel services. If your child does not have insurance, we can help! Your family might qualify for Medicaid/CHIP.
What is Medicaid?
Medicaid is a government health cover for eligible people, including many children and families. Eligibility rules vary by state.
What is CHIP?
CHIP stands for the Children's Health Insurance Program, which covers children under 19 in families who earn too much to qualify for Medicaid but still need affordable coverage.
If you do not qualify for Medicaid or CHIP, you may still have options.
- Visit healthcare.gov to shop for private marketplace plans. Depending on your income and household size, you may qualify for financial assistance that lowers monthly premiums and out-of-pocket costs.
- Community Health Centers — Federally Qualified Health Centers (FQHCs) offer care on a sliding fee scale based on your income. Find one near you at findahealthcenter.hrsa.gov.
