How it works
With Hazel, we’ll support you every step of the way
Parent or caregiver gives consent
Students can begin using Hazel services as soon as their parent or guardian completes our consent form.
*Students 18+ can refer themselves
Initial consultation
School staff or a parent requests an on-demand visit. Within minutes, the student is seen by a Hazel medical provider, and treatment recommendations are provided.
Post-visit summary
A discharge summary is sent to the parent with details on the care that was provided and the treatment plan.
Our team of licensed pediatric clinicians are on call for students, at school or at home.
