How Hazel online therapy works
All Hazel therapy sessions are conducted online via any smartphone, tablet, or computer that connects to WiFi. Sessions occur at school or home, depending on your school district’s program and family preference. The Hazel online therapy program is a short-term service.

Initial intake assessment (75m)
The intake session is your child’s first appointment with a Hazel therapist. During the session, the therapist will work to understand your child’s unique mental health needs.
As the guardian, you are expected to participate in this session virtually for at least the first 30 minutes.
Hazel sessions, including the intake must occur in a location with a physical address. Please do not join the intake session from a vehicle.

Weekly therapy sessions (45m)
After intake, your child can look forward to weekly counseling sessions, typically lasting six weeks to two months, subject to clinical judgment.
Hazel’s therapists lead students through an evidence-based treatment plan customized to their unique needs.

Visit the link below, enter your school district, and confirm that your school offers Hazel Health services.
Create your Hazel Health account
Start by setting up a secure parent or guardian account online. Here, you’ll be able to manage your services and keep in touch with your Hazel team.
Request online therapy for your child
Once logged into your Hazel Health account, you can click "Request Online Therapy" to complete steps required to make a referral.
Give consent for your child to receive care
Fill out and submit the required consent form for your child or teen within your account.
Schedule intake call and verify insurance
Once prompted, sign up for an enrollment call to schedule an intake and verify insurance information; otherwise, a Hazel Health representative will contact you within 2 business days.

Online therapy at home
For at-home sessions, an adult must be at home with your child in case of an emergency.
Guardians can connect to sessions by logging into their account.
Best practices for at-home online therapy
- Find a quiet place with no distractions
- Make sure your WiFi connection is strong
- Test your camera and microphone before the visit

Online therapy at school
A trained school staff member will help your child initiate the intake visit on the secure Hazel iPad. They will maintain ‘line of sight’ in case of an emergency while honoring the student’s privacy.

Insurance information
A member of the Hazel enrollment team will contact you to answer any questions and schedule your child’s intake appointment.
Before the call, please complete all enrollment steps in your Hazel account, including entering your child’s insurance information. Having your child’s insurance ready will help us get your student connected to care as quickly as possible.
We recognize that not everyone has insurance and therefore do not require insurance if you do not have coverage.
There is no out-of-pocket cost for Hazel Health services.
How to cancel or re-schedule an online therapy appointment

To help us serve another child, please message your student's therapist through the guardian portal or call Hazel at 1-888-541-7063 at least 24 hours before to cancel or reschedule.
We will work with you to find a convenient time for your child’s rescheduled visit, but care may be delayed, and preferred time slots may not be available.
By canceling or rescheduling 24 hours in advance, you enable us to help another family.
Services will be discontinued, and the student will be asked to re-enroll at a later day if that student has:
- 2 consecutive at-home visits canceled or rescheduled fewer than 24 hours in advance of the originally scheduled time OR
- 3 total at-home visits canceled or rescheduled fewer than 24 hours in advance of the originally scheduled time OR
- 1 week has elapsed since the date of a missed intake/counseling session, and no guardian has followed up
At a later date, students may request re-enrollment in the program as long as they renew a commitment to attending sessions as scheduled.
