Hazel Health visits may count toward a child’s insurance deductible. Hazel cannot verify individual insurance coverage or predict whether cost-sharing, such as co-pays or co-insurance, may apply. Families should contact their health plan directly to understand coverage and any potential financial responsibility for services.

Hazel Health submits claims to the child’s health plan for services provided. Depending on the health plan, families may be responsible for cost-sharing amounts determined by the plan. No further action is needed from families if a claim is denied, though they may wish to follow up with their health plan regarding any charges.
