Why is insurance information required for a Hazel visit?
Hazel Health has partnered with your school to help cover the cost of services. We submit claims to your child’s insurance to offset the cost of the visit. Depending on the child’s insurance plan, families may still be responsible for co-pays, co-insurance, or other cost-sharing amounts.
