What is Hazel’s role under SBHIP and CYBHI?

Hazel Health is an enrolled, credentialed CYBHI affiliate provider operating under California’s Student Behavioral Health Incentive Program (SBHIP) framework. Hazel delivers behavioral health services directly to students and, when eligible, bills insurers through the CYBHI Fee Schedule for those services.

The district is not a CYBHI billing entity for Hazel-delivered services.
