When a student's needs extend beyond what Hazel's program provides, we don't just discharge them — we help connect them to the appropriate next step. Our care team will provider referrals to vetted, qualified providers in the student's area, or to virtual providers if that's a better fit for the family. Our goal is to make sure no student falls through the cracks during a transition. Whether a student needs ongoing therapy, specialized treatment, or other long-term support, we work with families to ensure there is a clear path forward so that care continues seamlessly.
