How Hazel Health Works

With legal guardian permission, we help your student get the health care they need so they can focus on learning.

Physical Health Services

Our medical providers address student health needs in minutes, so they can return to class. If needed, our medical providers can send prescriptions to your local pharmacy.

Mental Health Services

Our licensed therapists help students cope with their feelings.

To get started, sign up, then make a referral by calling 404-383-6808.

Care coordination

If needed, we can help connect your student to ongoing care. This might include connection to a local therapist, a primary care provider, or a specialist.

You can refer your student for therapy, here's how:

  3. Fill out the form and click Submit Consent
  4. After you submit the form, create an account
  5. Request therapy for your child by calling Hazel Health at 404-383-6808.
  6. Within 2 business days, a Hazel Health representative will contact you to schedule an intake visit and verify insurance information

Through your account you can reschedule visits, send a message to Hazel, view upcoming appointments, and join scheduled therapy visits.

Health care at school or at home

When a student visits the health office, school staff use an iPad to help connect them with a Hazel provider or join a scheduled therapy appointment.

1. Sign up & submit insurance*

The legal guardian gives permission for the student to use Hazel. For mental health services, the student must be referred to therapy after the legal guardian completes the sign-up process.

*If the student does not have insurance, they will still receive care at no cost.

2. Student visits school health office

The student visits the school health office.

3. Virtual physical health visit or therapy session

A trained school staff member helps the student start a Hazel physical health visit or join a scheduled therapy appointment.

4. Follow up and care coordination

After the visit, Hazel provides treatment recommendations, follows up with the legal guardian, and connects the family to local care or resources when needed.

When a student needs care from home, the legal guardian can request a physical health visit, or help the student join a scheduled therapy appointment.

1. Sign up & submit insurance

The legal guardian gives permission for the student to use Hazel. For mental health services, the student must be referred to therapy after the legal guardian completes the sign-up process.

*If the student does not have insurance, they will still receive care at no cost.

2. At home virtual physical health visit or therapy session

The legal guardian requests a same-day physical health appointment, or helps the student join a scheduled therapy appointment.

3. Follow-up and care coordination

After the visit, Hazel provides treatment recommendations, follows up with the legal guardian, and connects the family to local care or resources when needed.

Submitting insurance & cost information

Hazel Health services are available at no out-of-pocket cost to families.

Please submit your insurance information during sign-up or by calling Hazel Health at 404-383-6808.

If you are insured, we will bill your insurance for services, like an in-person doctor's office would. There will not be a deductible or co-pay. No insurance? Your child can still receive care at no cost. Just confirm your insurance status at sign-up.

Hazel Health services are provided at no out-of-pocket cost to families.

Hazel Health bills insurance for Hazel services. This ensures that the visit cost is covered by insurance, and there are no out-of-pocket costs for the family. Having insurance information also helps Hazel to better coordinate care, such as referrals and prescriptions, to provide integrated physical and mental health care for students.

Over-the-counter medication administered during a Hazel visit at school is provided at no cost. If a Hazel provider recommends a prescription from a pharmacy or a referral to an outside provider, the family is responsible for any out-of-pocket costs.

Any student, regardless of insurance status, can use Hazel Health. Hazel will review and confirm the student’s insurance status when a visit is scheduled or delivered. If a student has healthcare coverage through a health plan or Medicaid, Hazel asks for that information to ensure the visit cost is covered by insurance, and there are no out-of-pocket costs for the family.
